The Personal Care Specialist supports the needs of home health patients by providing aide according to the patient's plan of care, while working under the direction of the assigned supervisor.

Essential Functions Statement(s)

  • Provide care based on patient's plan of care or service plan.
  • Provide or assist with personal care, including but not limited to bathing, dressing, oral hygiene, hair care, meal preparation, feeding, transfers and exercises based on the plan of care or service plan. Private duty includes pet care, housekeeping, errands, shopping and other client assistance services.
  • Maintain comfortable clean environment by adjusting lighting, room temperature, changing bed linens and light housekeeping as needed, paying close attention to safety factors and accident prevention in the client home.
  • Participate as a member of the interdisciplinary team and communicate changes in mental or physical status or other updates to appropriate individuals.
  • Follow individualized plan of care, documenting comprehensive observations, and interventions in the EMR and communicate as necessary with the appropriate individual.
  • Participate in continuous quality improvement activities.
  • Precept and train new staff, supporting professional relationships, as required.
  • Adhere to standard precautions and transmission-based infection control procedures, utilizing appropriate Personal Protective equipment
  • Follow all UCH policies and procedures as well as all applicable regulatory guidelines.
  • Participate and comply with all deadlines for education requirements, employee health screenings, and attendance at meetings.
  • Perform all other duties as assigned

SKILLS & ABILITIES Education / Certification / Licensure:

High School Graduate or General Education Development (GED): Preferred State Tested Nursing Assistant with the State of Ohio or the successful completion of the Certified Home Health Aid classes and certification exam CPR Certification Experience: One (1) year previous experience preferred. Computer Skills: Must be able to proficiently use a computer, the Internet and basic office equipment; Familiarity with Microsoft Office Software. Other Requirements: Must have a valid driver's license. Must be able to effectively perform essential duties while supporting UCH's mission, vision and values, in a professional and ethical manner. Must be able to adhere to all terms and conditions set forth in the United Church Homes Employee Handbook.
