POSITION OVERVIEW

Responsibilities include but may not be limited to:

• Transporting residents to and from medical trips, to and from SFR's or Program Centers, driving Merakey AVS buses and Vans.
• Inspect vehicles daily for cleanliness, routine maintenance, bodily damage and general appearance, notify HSS or designee of problems, be aware of routine and scheduled, mechanical maintenance.
• Accompany individuals to medical appointments as assigned.
• Maintain trip logs on a daily basis.
• familiarizes self with doctors, clinics, and various hospital environments.
• Reports to appropriate appointments on time and informs the parties of any pertinent information.
• Develops and maintains professional rapport with personnel of doctors' offices, clinics and hospitals.
• Maintains confidentiality.
• Verbally reports to HSS, Assistant HSS or SFR nurse upon return.
• Assists the residents with activities of Daily living, such as toileting, eating and other personal hygiene.
• Administers medication to clients while on medical trips, maintains Medication Administration Certificate.
• Performs related work as required. Assists with duties as directed by HSS/AHSS. Duties are subject to change.
